Asrani SG, Wilensky JT. Management of bleb leaks after glaucoma filtering surgery. Use of autologous fibrin tissue glue as an alternative. Opthalmology 1996;103(2):294-298.
Summary: Various methods of treating bleb leaks were studied, including the use of autologous fibrin tissue graft (AFTG) prepared by ammonium sulfate preciptation from 10ml of the patient’s blood. Nine of twelve of the patients treated by AFTG experienced successful healing, although there was no significant difference between AFTG application and other forms of treatment. In spite of this, the authors concluded that AFTG treatment is at least as efficacious and may, in some ways, be superior to other nonsurgical treatments.

Summary: Autologous fibrin concentrate injection into blebs of patients with persistent hypotony after glaucoma filtration surgery. Within 2 weeks, macular edema and visual acuity improved in six eyes (85.7%) and the trabeculectomy procedure remained successful for all eyes at a mean follow-up of 25 months.
Quotation: “We performed a simple office procedure by injecting AFC under the conjunctiva of the trabeculectomy site with topical anesthesia. This procedure reduced the function of the overfiltration bleb, improving both the hypotonous maculopathy and visual acuity. In addition, it did not cause corneal blood staining. On long-term followup, it was further noted that there was no excessive conjunctival scarring and bleb function loss.”
